In addition to residential support staff duties, employees in this category, under the supervision and direction of the Home Supervisor, will assist in the completion, maintenance, availability and retention of forms required in the daily operation of the home.
Include the following, other duties may be assigned
Maintains daily log and bed checks.
Maintains cleaning logs (three shifts).
Maintains food acceptance sheets (name, case number, diet), as needed.
Maintains inventory of Home medications.
Maintains activity calendar.
Maintains telephone logs.
Maintains vehicle mileage logs.
Maintains staff sign-in sheets (removes old, puts in new).
Maintains fire drill sheets (files in the log).
Keeps progress notes current in the charts (name, case number).
Keeps program sheets in the charts.
Orders and picks up forms from CMH and/or MOKA.
Reinforces torn or ripped sheets in the charts and logs.
Maintains corporate inventory.
Records and maintains staff meeting minutes.
Records and maintains inventory of nonprescription medical supplies (Band-Aids, gauze, tape, etc.).
Records and maintains inventory of rubber gloves.
Maintains emergency drills (fire, tornado, etc., and puts in the log for the month).
Maintains clothing inventory (forms handed out every three months to staff, January, April, July and October).
Makes sure that the physician order sheets and physician progress notes forms are plentiful in the residents' charts.

Education and/or Experience
That normally acquired in a high school diploma or GED.
Certificates, Licenses, Registrations
Must possess a valid MI Driver's license.
Ability to drive a 3/4-ton vehicle for transporting individuals served. Must possess good moral character as defined by the state of Michigan, and be at least 18 years of age.
Exerting 50-120 pounds of force occasionally, (when lifting an individual served in cooperation with at least one other employee), or in excess of 50 pounds of force frequently, or 10-20 pounds of force constantly to move objects or assist clients. Physical demand requirements are in excess of those for Medium work.
Are normally in an indoor residential setting, may include other indoor areas such as offices, stores, locations of social activities etc. May include exposure to outdoor atmospheric conditions experienced home maintenance, transporting or outing activities.

MOKA is a nonprofit charitable agency serving individuals with disabilities in a six county area in western Michigan. Founded in 1978 by a small group of concerned parents and citizens, MOKA provides critical supports for over 800 individuals and their families and employs approximately 580 staff members in support of this mission. Our agency provides a variety of services including specialized residential supports in community based homes, community living supports geared toward independent living, skill building programs based entirely in the community, supports coordination for individuals living in residential programs and in more independent settings as well as clinical supports (psychological, psychiatric, nursing, OT, PT, dietary, etc.) MOKA's Youth Autism Program works with children from the ages of 18 months to 18 years. Their work involves direct implementation of applied behavior analysis (ABA) principles under the clinical supervision of a board certified behavior analyst (BCBA). MOKA also provides social coaching for individuals with Asperger's Syndrome. Life Goal Planning, an empowerment tool created by MOKA is provided to interested individuals and their family as a way to identify critical steps in working toward goals of employment, independent living and building critical relationships needed to achieve success.
